Job Description

Overview Job Title: Post Lung Transplant Coordinator Department: University Hospital | Transplant Center Responsibilities Coordinate follow-up care for heart and lung transplant recipients after transplantation. Act as liaison between transplant surgeons/physicians, nurses, pharmacies, laboratories, home health agencies, and other support services. Provide patient education as indicated. Demonstrate knowledge and skills to provide age-appropriate care and to assess data reflecting patient status and collaborate with the transplant team to determine an appropriate course of action. Qualifications Minimum Qualifications: Graduate from an approved school of nursing. Current Ohio License. BSN preferred. Prior transplant experience preferred. Current Certified Clinical Transplant Coordinator (CCTC) certification preferred. Flexible, sensitive and adaptable to transplant activities. On going: Maintain current Ohio Licensure, CPR certification, and CCTC certification. Must be customer service oriented, dependable, self-directed, independent, organized and detail-oriented.
